I don't have specific article citations onhand, but 5 on-set accidents
come to mind:  Vic Morrow (father of Jennifer Jason Leigh) & stunt crew
members, on the set of TWILIGHT ZONE: THE MOVIE (1983) (see his imdb bio
at http://us.imdb.com/name/nm0607558/bio ); stunt performers on the
films THE SQUEEZE (1987), VAMPIRE IN BROOKLYN (1995), and GONE FISHIN'
(1997); and Brandon Lee's ( http://us.imdb.com/name/nm0000488/bio) death
on the set of THE CROW (1994).   Below are imdb links referencing the
stunt accidents in the last two of those films.  There was a lot written
about Morrow's and Lee's deaths, if you dig around for it.  Sorry I
don't have anything more specific; hope that helps a bit, though.
