I looked at a pre-recorded copy of Jamon Jamon yesterday. It does have a
picture of a woman with her legs wrapped around the waist of a man, however
in this version of the video cover the image was on the back not the front.
Given that it also has a word (which translates into Ham if I'm not
mistaken, which is quite apt as one of the protagonists is killed with what
looks like a leg of pork) repeated twice in the title it would seem to be
the film that you are looking for.

The film was released in 1992, the director was Jose Juan Bigas Luna and
starred Stefania Sandrelli, Penelope Cruz, Anna Galiena and Juan Diego. The
erotic black comedy-melodrama revolves around the class divisions between
two families when a girl becomes pregnant by her boss' spoiled son. It is
currently being screened by the satellite channel *Film Four* in the UK.
