I'm planning to conduct a survey to determine which audiovisual equipment are

used most frequently in the classroom.  If you or anyone at your college has

conducted such a survey in the past, I would appreciate receiving a copy of the

questionnaire used.  I would also appreciate any comments concerning things you

did or wish you had done, which you feel could improve the quality of the data
