You might want to try John Mueller who was and still probably is at the
University of Rochester. He had a wonderful Dance on film collection he would
rent out.

You also might want to try the Cunningham Dance Foundation just in case they
have a copy (the phone number long ago was 212-255-3130) since they have a
large film and video collection.

Also, I don't know if they have a collection, but the National Dance Museum
(?) in Saratoga Springs, NY might have some information on where to get a
copy.
