In addition to seconding the importance of Krin and Glen Gabbard's
book as well as Janet Walker's, I'd like to suggest one more film
I'm writing about that does interesting things with client-therapist
interactions--_Dancing in the Dark_ (1986).
 
The film is a combines voice-over narration of the journal
protagonist Edna compulsively fills (in an institution) with
flashbacks to domestic scenes.  Though there are several
client/therapist interactions, Edna is only once filmed in the same
frame as her male therapist.  This scene is also the only time Edna
speaks to him.  The first time we see him, it is in subsequent
parts--shoes, hands, and mouth--from Edna's point-of-view shot.
