I, too, like the Hillis Cole book.  Part II is formatting for television
formats.
 
Another, newer book is:
  Rieichman, Rick
     Formatting your screenplay.  New York: Paragon House, 1992.
 
$10.95 in paper copy.
 
It covers the same points and the Cole book, but is a little more
chatty.
